I accept Medical Mutual and Anthem Insurance. If I do not accept your insurance, payment is expected at the time of service either by Check, Cash, Mastercard, Visa, and Discover.  

I encourage all clients to inquire whether out-of-network services are available to you through your insurance if your insurance is one that I do not accept. If your insurance offers out-of-network benefits, you would pay at the conclusion of your session and I would supply you with needed information to be reimbursed for the session through your insurance company. 

All cancellations must be made ONLY by phone. All cancellations must be made 24 hours prior to the appointment time or cost of services, $65, with be charged.
